After you have filled out the forms you can file them at the Miami County Clerk's Office, 25 N. Broadway Rm 208. The cost to file the Name Change is $157.00, this can be paid in cash or by debit/credit card. You will be given the date of the Name Change hearing when you file the case. Name Change Petitions must be filed as a legal notice in the local newspaper 3 times before your hearing. You will be given the information to publish when you file, after you receive that you contact the newspaper and make arrangements to publish, this is an extra cost. It will be your responsibility to have the Notice published according to the Indiana Statute.
