IDOH has also partnered with Optum Serve to provide test sites across the state. Registration for testing at an Optum/LHI site is required and can be completed at https://lhi.care/covidtesting or by phone at 888-634-1116. Hoosiers will not be charged for testing, and insurance is not required. If you have private health insurance, please bring that information with you.
