Hours 8:00 AM to 12:00 PM and 2:30 PM to 3:30 PM

Maconaquah Schools: March 18 thru March 22

Peru Community Schools: March 25 thru March 29

North Miami Schools: April 1 thru April 5

All Schools: July 16 thru July 18

Eligibility Requirements

1) Un-insured (Does not have Medicaid or any other insurance)

2) Medicaid Only (With no primary insurance or primary insurance does not cover vaccines. A letter from your insurance provider is required)

3) Under-insured (Primary insurance does not cover vaccines. A letter from your insurance required)
