Services and Fees
 

The Miami County Registrar's Office performs three primary services for the public; issuing birth certificates, issuing death certificates, and performing genealogy lookups.

If you want to utilize these services through the mail, you will need to fill out the appropriate form, which can be found here.  Our forms require a PDF reader to view or print them.

These services can not be performed online.  Emailed requests for these services will not be fulfilled.

Effective January 1, 2008 the following rates apply to our services:

	Birth certificates - $10.00 per copy for walk-in or mail-in requests
	Death certificates - $10.00 per copy for walk-in or mail-in requests
	In-office paternity affidavit - $10.00
	Paternity upon marriage - $5.00
	Extra copies of any paternity - $1.00 per page
	Genealogy Search - $25.00 for first 1/2 hour search per name $10.00 for every 1/2 hour after that per name. (5 names per customer a day if time allows)

Warning: false application, altering, mutilating, or counterfeiting Indiana birth certificates is a criminal offense under I.C. 16-37-1-12.
