Our office is open from 8:00 a.m. until 4:00 p.m. on Monday through Friday. 

 

Please call 765-472-3901 extension 216 to set an appointment.

  • TB testing is done on a walk-in basis between 8:00 a.m. and 9:00 a.m. on Mondays and Wednesdays.

  • Adult Vaccines are given on Monday, Wednesday, and Friday by appointment only from 8:00 am to 3:30 pm.

  • Children's Immunizations are given on Tuesdays and Thursdays by appointment from 8:00 am until 3:30 pm.
