Each application must be accompanied by a certified birth certificate or
previous United States Passport, and two passport photos. A valid drivers
license must be presented as proof of identity at the time of application.
Per Public Law 106-113, Section 236, when applying for passports for children
under the age of 14, both parents or legal guardian must appear or documentation
must be submitted as to why that is not possible.
All applicants under age 14 must appear in the Clerk's office and be accompanied
by both parents to process the passport application. This change took effect
2/14/04
Children age 14 through 17 must appear in person accompanied by a parent or
legal guardian with certified documentation.
