Passport Applications
 

Photo Credit and Copyright: Jason StoutProcessing passport applications is not a required function of the Clerk’s Office. It is a courtesy we perform for the convenience of the public and our instructions are in the following paragraphs.

Passport applications are available from the Miami County Clerk’s Office, Room 208 from 8:00 a.m. to 4:00 p.m., Monday through Friday except holidays. Or you may go to the U.S. Department of State's website to download an application.

Once you have completed the application and turned it in to us, we will forward it to Passport Services for issuance. There is an approximate waiting period of 8 weeks before you will receive your passport through the mail. Passports are not issued by the Clerk’s Office.

Photo Credit and Copyright: Gail StoutEach application must be accompanied by a certified birth certificate or previous United States Passport, and two passport photos. A valid drivers license must be presented as proof of identity at the time of application.

Per Public Law 106-113, Section 236, when applying for passports for children under the age of 14, both parents or legal guardian must appear or documentation must be submitted as to why that is not possible.

All applicants under age 14 must appear in the Clerk's office and be accompanied by both parents to process the passport application. This change took effect 2/14/04

Children age 14 through 17 must appear in Photo Credit and Copyright: Global Marketing Enterprises www.globalme.bizperson accompanied by a parent or legal guardian with certified documentation.

There is a $30 cash execution fee for the Clerk's Office for each passport application, regardless of age.

Additionally, the fee for each applicant age 16 or older is $67.00 and this must be in the form of a check or money order made payable to US Department of State. Your passport will be valid for 10 years.

The fee for applicants under the age of 16 is $52.00 and must be in the form of a check or money order made payable to US Dept. Of State. These passports are valid for 5 years.

There is a different application for those wishing to renew an existing passport, but certain conditions must be met in order to use this different form. Complete the following checklist to determine your eligibility. If you answer "No" to any of the four statements below, you cannot use the renewal form.

 
I can submit my most recent passport.
I was at least 16 years of age when my most recent passport was issued.
I was issued my most recent passport less than 12 years ago.
I use the same name as on my most recent passport, or I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.
 
Important Changes:

Beginning January 23, 2007 all persons, including U.S. citizens, traveling by air between the United States and Bermuda, Canada, the Caribbean, Central America, Mexico and South America will be required to present a valid passport, or other approved document.


As early as January 1, 2008, all persons, including U.S. citizens, traveling between the United States and Bermuda, Canada, the Caribbean, Central America, Mexico and South America by land or sea (including ferries) may be required to present a valid passport, or other approved document.
